GLASS DOCTOR OF LEXINGTON — TERMS & CONDITIONS

1. WARRANTY. Glass Doctor warrants all workmanship for two (2) years from the date of installation. Manufactured products (IGUs, shower hardware, mirrors) carry their respective manufacturer warranties as separately documented. Warranty does not cover acts of God, vandalism, structural settling, or customer-caused damage.

2. PAYMENT TERMS. A 50% deposit is due upon estimate acceptance. The remaining balance is due upon job completion. Late payments accrue 1.5% interest per month. Returned checks incur a $35 fee.

3. CHANGE ORDERS. Any modification to scope, materials, or dimensions after estimate acceptance requires a signed change order and may affect pricing and timeline. Verbal change requests are not binding until documented.

4. LIEN WAIVER. Upon receipt of final payment, Glass Doctor will furnish a conditional waiver and release upon final payment per KY KRS 376.080 et seq.

5. CUSTOMER OBLIGATIONS. Customer agrees to provide reasonable site access, working power and water if applicable, and to clear furniture and decor from the work area prior to crew arrival. Glass Doctor is not responsible for damage to items left in the work area.

6. CANCELLATION. Customer may cancel within 3 business days of signing for a full refund. Cancellations after 3 business days but before materials are ordered incur a 15% restocking fee. Cancellations after materials are ordered are non-refundable for materials cost.

7. ACCEPTANCE. Signature below indicates customer acceptance of the scope of work, pricing, and these terms in full.
